About Us

Let me introduce myself, my name is Sammi, and I am a family nurse practitioner. I knew when I graduated I wanted to bring access to care to my community. I have a job that I love in family medicine. However, I have continued to have the desire to bring services to our area. Thus, Revive was born.
Seeing my dream come to life has been an amazing experience.
Here at Revive, will be offering botulinum toxin injections, lip filler, lip flips, IV Hydration, micro-needling and more.
